Children and Adolescents who benefits Home Support Teams (EAD-IA – CCISM)
Amount paid by the Health Service plus the maximum contribution paid by Social Security, per user/day, to the entities promoting and managing the units and teams of the National Network for Long-term Integrated Care – Rede Nacional de Cuidados Continuados Integrados (Public Entities of the National Health Service, Private Social Solidarity Institutions – IPSS or legally equivalent and Private Entities), with programme contracts, which develop Home Support Teams – Equipa de Apoio Domiciliário within the scope of Integrated Continuing Mental Health Care – Cuidados Continuados Integrados de Saúde Mental.
The National Network for Long-term Integrated Care – Rede Nacional de Cuidados Continuados Integrados (RNCCI) consists of continuing healthcare and/or social support units and teams, and a set of integrated continuing mental healthcare units and teams, which originate from local community services, covering hospitals, health centres, social security services, entities form social economy and local authorities. The RNCCI is implemented by the Ministries of Health and Labour, Solidarity and Social Security.
The Home Support Teams – Equipa de Apoio Domiciliário (EAD), promoted within the scope of Integrated Continuing Mental Health Care – Cuidados Continuados Integrados de Saúde Mental (CCISM), carry out the necessary activities in order to:
a) Maximise the autonomy of people with psychosocial disabilities;
b) Strengthen their social support network by promoting meaningful interpersonal relationships;
c) Improve their social integration and access to community resources;
d) Prevent hospital admissions and admissions to residential units;
e) Flag and refer situations of decompensation to the SLSM;
f) Support the participation of families and other carers in providing care at home.
